Care Connect

The challenge at hand
In 2023, Care Connect - Australia’s largest independent provider of care management services for older adults - was looking to renew its strategic plan for the next three years.
At the time, significant reforms to the broader aged care sector as a result of the Royal Commission into Aged Care Quality and Safety were about to come into effect. Preliminary government insights indicated that Care Connect’s current service model would be significantly impacted by these reforms, including price caps on care management fees and the introduction of a new funding model which would impact their revenue streams.
In light of these incoming reforms, Care Connect was looking to articulate how the organisation would position itself long-term and create a plan that was adaptive to future reforms as opposed to reactive.
How we helped
With a track-record of developing fresh and future-proof strategic plans, Impact Co was engaged by Care Connect to develop its 2023-2026 Strategic Plan.
To develop a plan that met the needs of Care Connect's key stakeholders, the project team facilitated a series of workshops with Care Connect staff members through which the key elements and parameters of the new strategic plan were discussed and validated.
Our engagement with Care Connect's people included one full-day workshop with 40 members of the Extended Leadership Team, a half day workshop with members of the Board and Executive Leadership Team and two online workshops with the Extended Leadership Team to validate the final content.
Impact
The final result was a 2023-2026 Strategic Plan which was endorsed by the Board and officially rolled-out across the organisation in July 2023.
